Patients are seen on an appointment basis by calling our office at (423) 246-4155.
If you cannot keep an appointment, please notify us so that we may utilize your appointment time for another patient. You will incur a no-show charge if you do not cancel your scheduled appointment.
On occasion, other patient emergencies can cause a delay in your being seen at your scheduled appointment time. Whenever possible, we will inform you and assist in making this inconvenience minimal. At these times, we appreciate your patience.
Kingsport Urology Group is a non-smoking facility. During your initial office visit, you will be asked to complete patient information forms to inform us about your medical history, to provide us with the needed information to allow us to contact you, and to assist us in working with your insurance company.
Please bring your medications or a current, accurate list of medications to each appointment. Please be sure to inform us if any of the initial information given has changed. It is important that we know if there are changes in your health, medication being taken, address, phone number, employment, marital status, and insurance coverage.
If your insurance company requires a referral from your primary care physician, it is your responsibility to make sure that we have the authorization before your appointment.
Due to frequent changes in insurance requirements, we will need to make a copy of your insurance card on each visit.
If you need to reach us after hours, please call our office and you will be directed to our answering service.
If an emergency urological situation should arise, we will do our best to respond to your need promptly. If the emergency occurs during office hours, please contact us as early in the day as possible. If the problem arises after office hours, please call our office and you will be directed to our answering service.
In the event of a severe situation, go immediately to one of the Emergency Rooms where a physician is on duty, and if our assistance is needed, the Emergency Room Physician will know how to reach our on-call doctor immediately.
All patients are encouraged to call with any urology questions. Our clinical staff are well qualified to answer most questions. Please call (423) 246-4155.
If the call requires the doctor to speak with you, he will review your medical record and return your call at the earliest opportunity.
Prescriptions and refills require a review of your medical record. Therefore, prescriptions and refills are only issued during office hours.
Prescriptions, to include pain medications, will not be reordered during the evening or weekends. To check the status as to whether or not your prescription has been called in, please check with your pharmacy after 4 pm.
If your medication requires authorization and form completion, you must obtain the appropriate form from your insurance company. We will complete the form in a timely manner.
Billing and Insurance
Co-payments and unpaid balances are due on the day of service. There will be a service charge assessed to your account for failure to pay your co-pay on the date of service.
Payment for professional services is your responsibility, not your insurance company. Even though we file insurance claims, you will receive a statement from us each month if there is a balance due.
We accept Medicare assignment and will file with your Medicare supplemental insurance. However, you are still responsible for any deductible or charges not covered by your insurances. After we have heard from all of your insurance plans, we will send you a statement for any outstanding balance due.
Please remember that no insurance company attempts to cover all medical costs.
Some companies pay fixed allowances for certain procedures and others pay only a percentage of the charge. As stated above, it is your responsibility to pay any deductible amount or any other balance not paid for by your insurance company.
We know questions can arise on insurance matters. We encourage you to discuss such questions with our business office staff.
Your medical record is confidential. It will not be provided to anyone unless we have written authorization from you. Please ask our Receptionist to provide you with a Medical Release of Information form, should you desire your record to be forwarded to another individual or company. A fee may be charged for these services.
We will be happy to complete one set of forms for FMLA, Return to work, Disability, Cancer Policy etc., at no cost to you. However, there will be a service charge for additional form completion for the same medical incident.
